INSTRUCTIONS FOR REQUESTING A RECORD BY MEMBERS OF THE PUBLIC

  1. In order to request a record an Open Records Request Form must be completed and submitted. Requests may be submitted either by mail, fax, email or in person.
  2. Please be sure to include your full name, address, daytime contact phone number, date of request and a brief, clear description of the information or record you are requesting. Unclear or incomplete requests may take longer to process.
  3. Depending on your request, you may be charged for copying costs and/or any staff time involved in obtaining the information. Possible charges and payment information is available here by clicking the Charges for Records Requests link.
